Please click on the links below to download an application form. We have provided one you can edit digitally and one you can print and complete by hand.

When completing an application, please note that the Doctor's signature must be current for the year in which the holiday is being taken.

At the end of the form, you will be asked to consent to us keeping your personal information on file and contacting you again in the future in line with our Privacy policy. Please read the Privacy policy carefully before submitting your application. While we will keep your contact details, we will destroy all medical data after one year and therefore a new application will be required each year.
